No two faces are alike.  Because of this simple fact, no two facial rejuvenation procedures are the same.  The principles of facial rejuvenation, however, are very simple and intuitive.  

5 Principles of Facial Rejuvenation:

The facelift technique used depends on your desired outcome, the health and condition of your skin, the degree of skin laxity, your lifestyle, age, and anatomy. Some individuals are of age and have skin laxity for a full facelift, while some may only be ready for a mid-facelift.

Ideal Candidates

The ideal candidate for a facelift or other facial rejuvenation surgery is a man or woman who is otherwise healthy.  

There is no specific age for when you would benefit from a facial rejuvenation surgery as the surgical technique depends more upon how you look as opposed to any specific age criterion.  For example, it is not entirely uncommon for us to see a woman in their 40's look like a woman in her 60's.   The aging process is obviously influenced by your genetics as well as environmental factors like sun exposure and smoking.

If you are in your 30's but concerned about mild facial laxity, you might benefit from some fillers to augment the cheek and temple in addition to a Quicklift procedure.  A patient in their 40's might benefit from a mini facelift but may also benefit from addressing their upper and / or lower eyelids.  A patient in their 60's most often will benefit from also addressing neck laxity as well as forehead droop.  

The surgical procedure for you will depend not so much on your chronological age, but rather upon HOW you are aging.  Patients should be physically healthy and be a nonsmoker, and also have expectations that are realistic.

Dr. Chang's surgical techniques are fully described in some of his published surgical videos.  Some of these videos are shown below.  Others can be found on the videos page.   These videos were recorded to demonstrate his technique to not only the general public but to visiting surgeons from around the world who aspired to learn his minimally invasive techniques which allow some of the procedures to be performed without the need for general surgery and with less recovery times.

Strategy for Treating mild facial droop ( potentially a patient in their late 30s or early 40s )

Quicklift + Facial Fillers or a Liquid Facelift 

A Quicklift is a simple procedure that uses a small incision in the temple inside the hairline.  The incision is used to gain access to the fibrous layer ( top-most portion of the SMAS ) underneath that has become loose.  Elevating this fibrous layer can elevate some of the cheek and some of the falling jowls.  This procedure is supplemented by adding some fillers to the under-eye hollows and sometimes the temples.

A Liquid Facelift is a simple procedure that is used for some patients with mild facial droop due to loss of facial volume only.  The facial volume is replaced with fillers like juvederm, voluma, sculptra, or your own fat ( called a fat transfer ).  The liquid facelift is also called a non-surgical facelift.  

Strategy for Treating mild to Moderate facial droop ( potentially a patient in their late 40s and 50s )

Most patients struggling with mild to moderate facial droop would benefit from a mini facelift.  This is a procedure that Dr. Chang has has adapted and has become an expert in the field among international surgeons.  The procedure can be performed without the need for general anesthesia in the office but you are also given the choice to have it done at the surgery center asleep.  This simple procedure has the goal of elevating the lower facial area including the jowls, nasolabial fold, marionette fold, and some upper neck laxity.  The incision is hidden inside the temporal hairline, descends inside but in front the ear where it is hidden, and then behind the earlobe.  After elevating the skin, the SMAS fibrous layer is elevated and sutured to the tough tissue along the cheekbone to give it long-lasting strength.    The procedure takes about 1.5 hours if it s performed alone; but most patients will choose to have additional procedures including their upper or lower eyelids, facial fillers, or fat transfer. 

Strategy for Treating Moderate to Severe Facial Droop That Usually Includes Neck Droop

Most patients with moderate to severe facial laxity also have neck laxity.  The neck muscles and skin begin to sag and sometimes even split giving the appearance of the turkey waddle.  These patients will need not only elevation of the face but will also need elevation of the neck.  The face portion of the procedure is performed similarly to that described above for the mini facelift.  The difference is that the incision is extended to the back of the ear and into the hairline behind the ear.  This procedure can also performed without the need for general anesthesia using our technique.   Patients in their 50s or older will most often also benefit from additional procedures including their upper or lower eyelids, facial fillers, or fat transfer.  Some patients may also benefit from a brow lift.  We will discuss whether you would benefit from being put to sleep at the surgery center

What to Expect

No matter which facelift technique you undergo, the primary goal is to produce a natural and soft result.   The procedure can be performed under general anesthesia or under oral awake anesthesia in the office.. The duration of the procedure depends on the invasive nature of the procedure, and also if other surgeries were combined. Following your procedure, there will be bruising and swelling, slight to moderate pain, and soreness, which can be eased by pain medication. Many patients feel and physically look better after several weeks. You can expect a more glowing look with less lines and sagging. Overall, the face will appear lifted and younger looking.

Should I get a surgical or nonsurgical facelift?
Although there are several great nonsurgical skin tightening treatments, these can only give you mild improvements. For moderate to severe skin sagging, facelift surgery is the best choice to improve your appearance. In your consultation, Dr. Chang will discuss your choices, including nonsurgical procedures, to help you decide which is best for your goals.

What about wrinkle fillers and relaxers over facelift surgery?
Wrinkle fillers and relaxers, like BOTOX and Juvéderm, have become more popular to improve fine lines and wrinkles. Also known as a liquid facelift, nonsurgical wrinkle treatments are good for early signs of aging; however, the results won't last more than 1 – 2 years.
