BOTOX can do much more than you have ever thought.  Many people don’t realize that many of the evolution of facial aging is caused by loss of volume in the face but also by how the face muscles try to compensate for this loss of volume.  For example, the horizontal forehead lines get deeper as the  forehead brow muscles work harder to lift the falling of the brows.

The horizontal forehead lines give the appearance of being worried, concerned, and older. They are caused by the frontalis muscle which normally act to raise the eyebrows.  

As you develop extra upper eyelid skin that can close off your eyes or as gravity drags your brows down with age, the frontalis muscle becomes hyperactive in an effort to raise the eyebrows so that you can see better. This effort causes the development of more and deeper forehead lines.  

Treating the frontalis muscle with botox will decrease the appearance of these lines.  

Be aware that this muscle normally keeps your brow elevated. Over-treatment of this area causes the brow to fall and the eyelids to feel heavy.  This is the most common problem we see from patients treated by less experienced doctors and practitioners from other facilities.  

We have tips and tricks to not only avoid this problem but to also fix the problem once we see it.

The vertical “angry” lines between the brows above the nose give the appearance of being angry or worried. The lines are caused by the glabellar and corrugator muscles which normally act to bring the grows centrally and down.  

Treating these muscles will not only reduce the appearance of these lines, but will actually elevate the brows

The crows feet lines are the wrinkles that extend to the side of the eye when you smile or squint. 

They are caused by a circular muscle around the the entire eye called the orbicularis muscle. Treating this muscle will reduce the appearance of the crow’s feet lines. 

The lateral brow lift is included here because it is one of the most popular treatments that we perform using BOTOX, even though it is not a wrinkle treatment.  

The procedure is performed by treating the upper part of the circular orbicularis muscle (see crow’s feet). Contraction of this circular eye muscle near the eyebrow causes the brow to come down. Treatment of this muscle will therefore raise the lateral brow. Similarly, treatment of the glabellar muscle (see the 11’s) which normally causes the medial brow to come down will raise the medial brow.

The result from this treatment leave the brow slightly higher at the corners helping to brighten up the eye itself. It leaves the patient feeling more awake and helps to prevent the need for upper eye lid surgery for some time. 

BOTOX has revolutionized how plastic surgeons keep patients looking younger for longer without the need for surgery.  BOTOX is most commonly used to remove wrinkles located on the forehead, between the brows,  and around the eyes. It is meant to slow down the aging process and prevent the break-down of facial collagen. BOTOX was FDA approved in 2002 to be most commonly used for what patients fondly refer to as either the “11s” or “angry lines”.  Prior to 2002 however, BOTOX was used in more “legitimate” medicine to treat muscles spasms. To this day, BOTOX is actually used more in medicine than in cosmetic treatments.

How does BOTOX take away wrinkles?

BOTOX is a neuromodulator which temporarily prevents affected muscles from contracting. The forehead lines, glabellar 11’s and the crow’s feet lines are all caused by hyperactive muscle action in the face of loose skin that results from loss of facial volume and the breakdown of tissue collagen. By temporarily preventing these muscles from contracting, the muscle is unable to cause the crease; and the crease will if it is not too ingrained, soften and then disappear. How Botox in the Forehead Slows Down Aging

Treating the forehead will not only reduce the lines of the forehead but because the muscles will not be able to contract, the skin collagen does not break down as fast. This will ultimately keep the skin from stretching or sagging with time, thereby preserving your brows where they are supposed to be.

Caution:

If you already have low brows, then relaxing the forehead muscles (which normally elevate the brows) will cause the brows to fall; giving you the feeling and expression that your brows are heavy.

Angry 11s Between the Brows

The vertical wrinkle between your brows have been given many names. These include the 11s or the “angry” lines. These muscles are called the glabellar muscles (the corrugator muscle and the procerrus muscle) run between the eyebrows to bring them down and toward the nose; and helps with facial expressions such as to express concern, worry, anger, or concentration… none of which are good. Treating these lines gives a strong impression of youth. Nobody wants to look concerned or angry.

How Botox Slows Down Aging

Treating the crow’s feet lines will not only reduce the visible wrinkles around the eye, but it will slow down aging in the area. Because the muscles is no longer able to contract the skin, the collagen does not break down as fast. The lower eyelid skin does not stretch or sag as fast over time.

Crow's Feet

The crows feet lines are the wrinkles that extend to the side of the eye when you smile or squint. They are caused by a circular muscle around the the entire eye called the orbicularis. Therefore, treating this muscle will only prevent the part of the muscle that is treated from contracting that is a good thing. As we get older, we begin to lose tissue volume in the temples. This allows the deflated tissue to sag; the orbicularis muscle becomes hyperactive because it has to work harder to move the skin that is no longer tight. A nice addition to this procedure is to add a cosmetic filler like Juvederm or Voluma to the temples.

A limitation to the treatment is BOTOX will not treat all the lines consistently. In particular, the lower lines on the cheek are not usually treated. These are caused more so by the cheek elevating when you smile as opposed to the muscle action of the orbicularis muscle  

How Botox in Glabella Slows Down Aging

Treating the 11’s will not only reduce the lines of the forehead, but because the muscles will not be able to contract, the brows will stay in a higher position over time. Not only are higher brows, more youthful, but they take the pressure off the upper eyelids.

Caution:

Some inexperience practitioners will get too close to the upper eyelid which has a different muscle that raises the upper eyelid. The BOTOX will stop the upper eyelid muscle from working which normally raises the upper eyelid. The result is and eyelid droop. Eyelid droop can be partially treated with iodipine drops.

Brow Lift

The lateral brow lift is included here because it is one of the most popular treatments that we perform using BOTOX, even though it is not a wrinkle treatment.

The procedure is performed by treating the upper part of the circular orbicularis muscle (see crow’s feet). Contraction of this circular eye muscles near the eyebrow causes the brow to come down. Bunny Lines

The Bunny Lines are wrinkles over the bridge of the nose as you wrinkle your nose or smile, They are caused by the nasalis muscles located over the bridge.

What To Expect

YOUR EVALUATION

4 pillars of facial aging

If you have never had BOTOX before, your evaluation and consultation will review the 4 pillars of aging. These include the loss of facial volume, the loss and break-down of tissue collagen, the resulting hyperactivity of the muscles of facial expression, and the surface skin changes that change the radiance of your skin. These are reviewed in a different page.  

YOUR ANTI-AGING PLAN

Based on how these changes may have affected you, we will develop a treatment plan. It is very possible that BOTOX is the only item on the treatment plan.  

YOUR BOTOX TREATMENT PLAN

Based first on your concerns, and then on your physical evaluation, a treatment plan is created just for BOTOX. This includes deciding whether you would benefit from treatment of your forehead, 11’s, crow’s feet, or bunny lines. Each area will generally require about 20 units of botox for optimal results. 

WHAT TO EXPECT

YOUR INJECTION

YOUR FIRST BOTOX INJECTION

Your treatment will be done by one of our cosmetic nurses who have been personally trained by Dr. Chang. Their training requires a full year internship. At the end of the internship, they would have had more experience than 90 percent of non-plastic surgeons and 70% of plastic surgeons. They are trained in surgery to see the real muscles in action that they will at some point address. 

Each area is treated with a 30 gauge needle. This is the smallest needle available provided for your comfort. Topical numbing is generally not provided as it is not particularly effective. You may, however, request it if you want. Each area will have 2-4 injections per side.  In general, there is no significant bruising. 

WHAT TO EXPECT

AFTER TREATMENT

AFTER YOUR FIRST TREATMENT

The reduced muscle activity starts 3-4 days after receiving your injections.  from that point, it will take another 2 weeks to see maximum benefit. Although we will often hear anecdotal stories of Botox lasting 6 or more months. The manufacturer reports an average of about 3-4 months. The strategy is to be re-treated in 3 months in order to avoid having the muscle get worked up and strong again.  

Your smooth, younger-looking skin will remain for approximately three to four months before the muscles slowly being to move again. Maintenance injections every three to four months will preserve your smoother, more youthful look.
