Aging in the neck is a continuation of aging in the face. After all, gravity takes everything downward. It can be covered by turtleneck sweaters in winter but come summer the neck is an extremely exposed part of the body and it is hard to cover-up.
Some features of the aging neck include:
- Accumulation of fat under the chin – can be known as jowling
- Sagging of the skin, muscle, and fat causing the “waddle”
- Splitting of the platysma muscle in the midline of the neck – this causes excess skin and wrinkles
- Lose of a distinct jawline
The neck is defined as everything between the jaw line and the clavicles. Neck lift surgery is a procedure to lift the sagging skin, muscle, and fat of the neck. The procedure can and is commonly performed in conjunction with a mini facelift, neck liposuction, and platysmaplasty, and will vary depending on which, if any procedures are added.
A mini facelift is a procedure to lift the lower face. The primary goals are to lift and remove any extra facial skin that contributes to sagging skin of the lower face, improve the jawline, and reduce the appearance of the nasolabial fold and marionette lines. A mini facelift alone will lift the neck by up to 5o%. Patients who don’t need or desire to lift the neck any more than that will not need a neck lift.
Neck liposuction is performed if there is isolated fat in the neck below the jawline. Fat in the neck not only camouflages the sharp angle between the neck and jawline, but the weight of the fat pulls the tissue down faster over time. Neck liposuction is performed first before the neck lift to debulk the fat and define the neck
Platysmaplasty is performed if the midline platysma muscles have split. The platysma muscles are usually joined in the midline in our youth and split with age in about 30% of patients. Once the neck liposuction is performed to debulk the fat, an incision is made in the chin crease beneath the chin. The split platysma muscles are identified, and joined by sutures in the midline again.
The neck lift is performed after the neck liposuction and/or platysmaplasty if the patient needed one or both of those procedures. It is performed by making an incision beneath the earlobe that extends in upwards behind the ear in the crease between the ear and the skull. Once the incision reaches the top of the ear, it is extended posteriorly into the hairline creating a hockey-stick appearance. The excess skin, fat, and platysma are elevated and tightened.
The procedure can be performed in the office under local anesthesia for selected patients and will take about one to two hours to complete. The majority of the visible sutures are absorbable.
A bulky headwrap is placed once the procedure is completed.
CoolSculpting is a non-surgical option to reduce fat in the neck. It is a great alternative to liposuction if a patient doesn’t require any other surgical procedures such as neck lift and platysmaplasty.
It can be done in office and there is no downtime. The fat cells are frozen and die, and the body naturally removes them. There should be a reduction of around 25% in the treated area.
There are a number of reasons people consider neck lifts. The most common causes are:
- you want a slimmer neck and to remove the fatty jowls that have appeared.
- You have excess skin that droops and sags
- You have developed folds and lines
- You wish to have a more distinct jawline
After surgery you should rest at home and let yourself heal. Dr. Chang will send you home with a detailed recovery plan. There are a number of different ways to help aid recovery:
- All patients wear a bulky headwrap for about 1 week.
- There will be some bruising and swelling which should fade within a couple of weeks.
- Most patients are presentable without drawing attention in 1 to 2 weeks.
- Sutures are removed 10 to 14 days after your surgery. Some may be absorbable.
- Strenuous exercise should be avoided during this time to help in the recovery process.
- Apply aquaphor ointment twice a day until the sutures are removed.
- Most patients will take one to two weeks off work.
- Pain is negligible. Pain medication will be provided for the first few days.
As with any surgery, there are associated risks. Only you can decide whether a neck lift can achieve your goals and whether the recovery and risk is acceptable to you. Complications are rare but can include:
- Local bleeding
- Facial asymmetry
- Infection particularly from the absorbable sutures
- Loss of sensation although rare
- Widened or thickened scar
- Delayed wound healing with skin or tissue necrosis
- Extra skin at the ends of the incisions
- For men, temporary or permanent hair loss where the incision is made
- Swelling which doesn’t go down
Unsatisfactory results may require additional procedures. Patients will generally be responsible for any associated facility fees.
Scarring will depend on the procedure. If you have Coolsculpting there will be no scar.
If you are considering surgery there will be a scar from the incisions, but Dr. Chang will ensure that they are non-visible either behind the ears or right under your chinline.
