How Does Sculptra Work?

Sculptra acts as a bio-activator and extremely powerful collagen stimulator. In addition to the volume of what is injected, it stimulates your own collagen, allowing tissue to volumize gradually. Over time, the Poly-L-Lactic Acid dissipates as your own tissue takes its place. Since the product triggers your body to build its own collagen, results are not immediate and can take anywhere from 4-6 weeks

How long does Sculptra Last?

Sculptra will last about 2-3 years depending upon the movement of the area.  It is more stable in areas that don’t move as much such as in the cheek mound.  It will dissolve quicker in areas that move a lot such as in the nasolabial folds and the marionette lines.  So while it is a bit more costly than H.A. fillers, patients often justify spending a little more for the time in which they can enjoy results. Most fillers only last a year at the most so Sculptra is considered a great investment.

What areas are most commonly treated with Sculptra?

Sculptra builds up collagen and tends to be less pliable than the hyaluronic acid fillers like Juvederm or Sculptra.  For this reason, it can not be used in lips.  Sculptra is most commonly used in the cheeks, temples, and chin.

How much Sculptra will I need?

The amount of Sculptra needed will depend on your individual features.  As a rule of thumb, most patients will benefit from at least 2-4 vials of filler.  Each cheek will generally need its own vial.  And each temple will generally needs its own vial also.   A second yardstick is one vial per decade of life. So if you’re in your 40’s, you might need 4 vials. There are exceptions to this rule depending upon volume loss.In general, we do not recommend more than 4 vials at a time.  If you need more, we will wait 6 weeks to see maximum visual benefit before proceeding with more treatments.  Most patients should maintain their results with a vial per year after their initial treatments are completed.

Procedure Technique

How is a Sculptra injection procedure performed?

  • Step 1 - Consultation about the potential benefits of volume replacement for you
  • Step 2-  Detail where you will benefit from volume replacement and supplementation
  • Step 3-  Choose the appropriate filler based on the area, duration of action, and complication profile.  Some patients will benefit from different fillers in different areas.  For example, if you want augmentation of the lips, under-eye hollows, the temples, and the cheeks, one treatment strategy could be to use sculptra for the cheeks and temples and juvederm for the under-eye hollows and lips.
  • Step 4-  Choose the volume required to provide maximum benefit
  • Step 6 - Inject the fillers based on your needs

Following your consult to develop your unique plan for treatment, you can receive your injections in our procedure rooms. A local anesthetic or numbing cream may be applied prior to sterilizing and marking the sites to be injected. As soon as you are prepared, Sculptra is injected down into the deeper skin layers at the chosen areas of the face. For longer-lasting results, Dr. Phillip Chang performs a series of four treatments that are each performed at Aesthetica Cosmetic Surgery & Laser Center around three weeks apart.

What to Expect

Following your appointment, you may experience mild or moderate swelling, redness, bruising, or tenderness at the injection areas that should resolve on its own in several days. To encourage healing, you may apply a cold compress to the injected areas. You should limit UV exposure after your appointment and always use sunscreen. Many patients will notice clear improvements immediately following their appointment with final results seen after a few months, when the increased collagen production improves the health of the complexion. If Sculptra is performed in a series over 3 – 4 months, a patient's results may be visible for as long as two years.

What to expect after your Sculptra treatment?

Minor to moderate swelling should be expected.  Some patients may return to work immediately but many will want to wait until the next day.  Final results will not be seen for up to 6 weeks.

What are the risks and complications of Sculptra ?

  • All injectable fillers have the potential to be lumpy or irregular
  • All injections have the potential for bruising or swelling.  
  • In VERY rare cases, the filler can put pressure on a blood vessel or a nerve.  
  • Sculptra has the potential for forming minor cysts and granulomas.

The most common age to receive Sculptra injections is between ages 30 – 55; however, it is also common for patients who are younger and older than this range to receive injections.

How long do results last?
For longer-lasting results, some patients may need to receive several Sculptra treatments. Sculptra is different from other fillers because the results usually last up to two years. Since the production of collagen continues to develop over time, results may appear more gradually.

Can you use different fillers at the same time?

Many patients will use the strategy of using different fillers for different goals. For example, patients who want a complete facial rejuvenation ( sometimes called a liquid facelift ) will have longer acting fillers like Sculptra placed in areas that need larger volumes but don’t tend to have as many problems; and softer and safer products like Sculptra or another HA in more sensitive areas like the under-eye hollows or the lips.

How do injectable fillers slow facial aging?

Injectable fillers slow facial aging by restoring volume to loose and sagging tissue. Think about a balloon, as the balloon is deflated, it becomes floppy; but as you refill and restore volume to the balloon, the skin becomes tense and full. Because the tissue is no longer loose, sagging, and mobile, the collagen does not break down as quickly. Tissue does not stretch or fall as quickly.

What is the longest lasting filler?

The longest lasting filler that we use is Sculptra. It lasts between 2-3 years depending on the volume injected into the area. Fat transfer can last 5 to 10 years but requires the need for recovery time.

What is the best injectable filler for underneath my eyes and cheek?

The best injectables for underneath the eyes are the ones that are safest and the softest. These include the hyaluronic gels Juvederm and Sculptra. Both of these are soft and tissue-like for the the skin in the area but can also be dissolved if you don’t like the results. I also like Radiesse if it is used more laterally to the side as it is moldable and actually builds up collagen. Juvederm will last about 8-12 months while Sculptra and Radiesse might last up to 2 years. Fat transfer with or without PRP therapy is my favorite as it directly replaces the lost fat but this requires the need for more recovery time.

What is the benefit of using fat transfer instead of injectable fillers?

Fat transfer with or without PRP therapy is the most natural way to provide or replace volume in the face. The procedure is performed by first taking fat cells from your abdomen, waist, or thighs; then processing the fat so we have only fat tissue, and then injecting it into the face wherever it is needed. The most important benefit is that it provides your own natural tissue. You are providing volume replacement with your own tissue. The tissue is soft and it is living. This is why it is usually difficult to answer the question about how long it will last. Because it is living tissue, it theoretically lasts forever. The problem lies more in the fact that we all lose more fat volume in the face with time and aging. The second benefit is that it is ultimately less expensive over time because you are not paying the overhead of injectable fillers. We do not have to pay a medical device company half of the syringe fee as you are providing your own fillers. Finally, you have unlimited supply.
