Forehead Lift (Brow Lift)

Do you look constantly tired even after a good nights sleep? Are you people asking if you are angry when you know you are not? Aging can cause the brow and forehead to droop which pushes the brows downwards and causes sagging and wrinkling.
The forehead lift procedure has the dual purpose of elevating the eyebrows while taking the pressure off the upper eyelids. The end result is a fresher, more youthful appearance.
There have been many types of forehead lifts historically. The open forehead lift was the first popular type followed by the endoscopic forehead lift. Both of these procedures had significant limitations.
Modern approaches and techniques have popularized our versions of the hairline forehead lift and the temple lift which minimizes the appearance of scarring, significantly decreases recovery time, all while producing longer lasting results.
The hairline forehead lift:
The hairline forehead lift is an adaptation of the open forehead lift. The procedure is performed by making a zigzag incision at the hairline. Because the incision is closer to the brow than the older traditional open forehead lift, this technique allows a more significant and stronger lift, and decreases swelling, while decreasing complications such as numbness and hair loss. The zigzag pattern allows the hair to fall between the zigs and the zags, effectively camouflaging visible scarring.
The temporal brow lift:
The temporal brow lift is similar to the hairline forehead lift except that the procedure is directed more at the lateral brow. The lateral brow falls more significantly than the central brow, probably because of loss of facial volume in the temples. The temporal brow lift uses a zig-zag incision either in the hairline of the temple area or slightly above it to camouflage any scar even further if possible.
Patients are given the choice to have the procedure in the office with local anesthesia and valium or at the surgery center for general anesthesia. The procedure will take about 1-1.5 hours. Patients will recover at home and return for a follow-up a few days after.
The goal is to reduce the visible signs of aging on the upper section of the face. The aim is to:
- Elevate the brows to a more attractive location
- Take the pressure off the upper eyelids
- Open up the appearance of the eyes
- Eliminate the constant tired, grumpy look
After surgery you should rest at home and let yourself heal. Dr. Chang will send you home with a detailed recovery plan. There are a number of different things to expect:
- There will be bruising for 2-3 weeks
- Final results should not be expected for 3-6 months
- Most patients will return to work in 1-2 weeks with make-up
- Sutures are removed in 7-10 days if they have not absorbed
- Avoid strenuous exercise for 2-4 weeks
- Apply scar cream 2x per day until completed
- Pain is negligible
- Scarring will be minimal and hidden in the natural skin folds or in the hairline
As with any surgery, there are associated risks. Only you can decide whether a forehead lift surgery can achieve your goals and if the recovery and risks are acceptable to you. Complications are rare but can include:
- Local bleeding or prolonged bruising
- Prolonged swelling
- Milia (small cysts along the suture line)
- Residual skin
- Infection particularly from the absorbable sutures
- Loss of sensation although rare
- Widened or thickened scar
- Asymmetry (unevenness between two sides)
- Delayed wound healing with skin or tissue necrosis
- Extra skin at the ends of the incisions
Unsatisfactory results may require additional procedures. Patients will generally be responsible for any associated facility fees.

Patients are often confused about whether they need a forehead lift or an upper eyelid blepharoplasty. Sometimes, they will need both. The forehead skin will fall with age. This will push pressure and close off the upper eyelids.
You can demonstrate this for yourself in a mirror. Place your hand on your forehead and push down. You will see that your upper eyelids close off. Now do the opposite. Lift your brows to what you think is an attractive youthful position. See if you still have extra skin on your upper eyelids.
You will benefit from a upper eyelid surgery alongside your forehead lift if you elevate your brows and the brow position looks attractive to you or if there remains extra skin on your upper eyelids.
