About Dermal Fillers

Young faces are round faces. Restoring the facial volume that is lost with age is the first step in helping you look younger. The most common areas to address include the temple as it extends down to the cheek mound, the under-eye hollows, and the tissue around the mouth including the lips. Restoring volume not only looks more youthful, but it will lift sagging tissue to some extent. This is the inspiration behind the liquid facelift procedure.  

Acne scars are extremely difficult to address. The treatment of acne scars often will require multiple methods together or in succession. Acne scars often appear as a crater or “ice pick” mark where the skin in tethered and pulled to the deeper tissues. The most common treatments performed include profractional laser therapy and using fillers

 

to elevate the cratered skin. Profractional laser therapy is used to shrink the tissue as well as promote collagen growth. The filler is placed between the cratered tissue and the underlying tissue.

The lips are the evolutionary centerpiece of facial sensuality. The curves, outline, and fullness of the lips embody a woman’s sexuality. The introduction of fillers such as Juvederm and Volbella have revolutionized the cosmetic treatment of the lips. These fillers are able to provide extremely soft gel fullness to the lips with (in the right hands) negligible side effects. 

The under-eye hollows show your age as much as any other aspect of aging. Volume that extends from the cheek mound up to the lower eyelid fades with age. The loss of volume results in deflation and sagging of the cheek; the result of the accentuation of the nasolabial fold as well as the outline of the orbital bone at the base of the lower eyelid. The lower eyelid will thin as the tissue is pulled lower, the skin will appear as dark circles, and the lower eyelid will appear longer and more wrinkled.  

These changes occur as early as your 20’s but accelerate with age. Restoring volume here with Juvederm or Voluma will not only recreate a youthful appearance but will slow many of the changes that were previously mentioned.

Hand rejuvenation is addressed by restoring volume with dermal fillers or fat, ligation of particularly large hand veins, and Forever Young BBL treatment to remove age and sun spots.

It is not a secret that skin appears to sag as you get older. Some of the sagging is due to the loss or relaxation of collagen, but much of the appearance of sagging is simply due to the loss of volume. This is also true in the breast, the buttocks, and the hands.  Restoring volume to these areas will partially lift up the tissue and should be performed before tissue excision to avoid a “pulled” look. Overfilling, however, should be avoided in order to avoid a distorted and unnatural appearance.

Which Fillers Are Right For You?

Look Younger Longer

Juvederm was one of the original dermal fillers. It is a hyaluronic acid gel that has become one of the most popular fillers on the market. Over the subsequent years, Juvederm has evolved in to a family of fillers that includes Juvederm Ultra, Voluma, Volbella, and Vollure. They each have different qualities and may be used for different purposes on the face and body. 

They are all soft gels that will last anywhere from 9 months to about 2 years depending on the filler.

Juvederm Ultra is the oldest of the hyaluronic acid fillers created by Allergan, who is also the manufacturer of Botox.  Juvederm Ultra has the shortest duration (9 months) of all of the fillers. Some might think of this as a negative, but this is not always the case. Because of its shorter duration and ease of use, it is possibly the best filler for you if you are unsure of the benefits and you want something that might dissolve more quickly.

Voluma was the second hyaluronic acid filler created by Allergan. Voluma was created to last up to 2 years. Although soft, it is slightly thicker than Juvederm Ultra and is therefore not as appropriate for the lips.

Volbella is a newer hyaluronic acid filler designed to treat sharp lines and wrinkles such as in “smoker’s lines.” This is made possible because it is more liquid than the other fillers.

Vollure is a new hyaluronic acid filler designed for areas with thin skin such as under the eyes. 

  • May be used for nasolabial folds and marionette lines if the skin is thin.

Sculptra is a cosmetic filler that we turn to in patients who need a lot of volume. You will get more volume per syringe than any of the other fillers. In addition, Sculptra will also last longer than other dermal fillers. Sculptra helps stimulate your skin’s own natural collagen production to help restore it’s inner structure and increase facial volume that has been lost due to aging.  

These qualities make Sculptra the most cost effective dermal filler that we use. This begs the question, why don’t we use Sculptra for everything? Sculptra requires more potential recovery than the other fillers because it tends to cause more swelling and bruising. It also cannot be used anywhere the tissue is thin, such as around the eyes or lips.

Also called a nonsurgical facelift, the liquid facelift is a quick procedure that uses cosmetic injectable fillers and BOTOX without surgery to give you a more youthful appearance. As previously discussed, volume loss will initiate the sagging of the face as early as in your twenties. Adding volume and inhibiting the dynamic muscles with BOTOX will within reason, lift the tissue.

Fat transfer is obviously the most natural of cosmetic fillers. The fat is taken from your own body from an area that you normally find difficult to get rid of, such as the lower abdomen, inner thighs, or flanks. It is then processed and transferred to the area which you are looking to provide extra volume. With the fat transfer, there is a significantly less overhead cost than using artificial fillers like Juvederm or Sculptra. Therefore, if you require a significant amount of volume, fat becomes the most cost effective option.

Fat transfer is most commonly used for the face, but also very common for breast augmentation, Brazilian butt lift, lips, and even hand rejuvenation. Fat provides a soft natural look and feel.

The downside is that it results in more swelling and bruising than the artificial fillers. Depending on its placement, some patients can have obvious swelling for up to two weeks with complete resolution in 1-3 months. In addition, not all of the fat will survive the transfer.

Discuss the areas to be addressed

As previously discussed, dermal fillers and/or fat transfer are used to add volume to areas of the face or body that either have lost or lack adequate volume. These areas can include the face (see details above), hands, breast, or butt. Depending on the area, a filler with the correct properties will be chosen.

Choose the right filler for YOU

Dermal fillers and fat transfer all have different properties. We will choose the correct option together based on the type of area, how long the filler lasts, and the price. In general, Voluma which lasts for about 1 and a half to 2 years, are used in the localized areas of the face except for the lips and under-eye are.  Those areas are treated with Juvederm (1 year) or a judicious amount of Voluma. Large surface areas are treated with Sculptra or a fat transfer. Sculptra will last about 2-3 years and fat may last a life-time.

Most facial fillers come with numbing medications in the syringe decreasing the pain of injection. Topical numbing is usually added to the lips as they are generally more painful. 

Most patients will not experience pain or bruising after their procedure. Most patients may return to work immediately especially if there is only one or two syringes used. The exception is patients who have injections with Sculptra as the procedure requires a larger needle and multiple passes. Most patients will still not bruise considerably. It’s rare for a patient to experience considerable bruising.

All patients can expect some mild swelling after their procedure. Most patients will not have any significant bruising. Swelling will decrease significantly in the first 2-3 days but you can expect some minimal swelling for up to 3 weeks. You should schedule your follow-up no earlier than 2 weeks after the initial treatment for this reason. 

If you absolutely hate your result, it may be dissolved using hyaluronidase for hyaluronic acid products. There is no refund as we have already used the product and the work is done. Sculptra cannot be removed.

Sculptra aesthetic is a poly-L-lactic acid injectable that helps produce collagen/volume into the tissue it was injected. 

Volume replacement in the butt requires a good amount of product. One of the most cost effective ways is a fat transfer. You also see the added benefit of liposuction where the fat was taken from. If someone doesn’t have enough fat to take, or if they want an injectable option, Sculptra Aesthetic is a good alternative to increase volume. 

Depending on the degree of severity of the sagging, a surgical butt lift can be done, or also volume replacement with either fat or injectable volume replacement. 

Yes, technically any filler can be placed in the butt, for for longevity sake and for the proper firmness and volume, we recommend the use of Sculptra.

Sculptra is indicated to last over 2 years, many people see a result that lasts upwards of 5 years. 

Sculptra helps your body produce its own volume, so with enough stimulation your body will produce and keep volume in the area for years. Most people do “touch up” treatments annually. 

Priced per consultation it really depends on how many vials of Sculptra we use. on average we will use 10-20 vials for a full butt. 

Finding a Board Certified Plastic Surgeon near you is easy with a google search. Seek providers who have good photos on their website and go in for a consultation first. After gathering information go with the provider that you feel comfortable with. 

Start with google reviews or word of mouth, and then check out their credentials too. It is important the a plastic surgeon be board certified and have experience.  

Lips are an art form. If you are looking for volume in the lips then Juvederm Ultra Plus XC works best. it holds a nice shape, and gives volume to increase your pout. 

We use a different filler around the lips than we do in the lips. Juvederm Vollure or Juvederm Volbella both work well for smaller finer lines around the lips without adding bulk volume to the area. They are both meant to fill in fine textural lines.

Filler under the eye should always be done by a very experienced injector. Certain products can cause under eye puffiness to become worse. Using a product that is less water loving is important. Volume in the cheek can also address under eye issues. We would use Juvederm Voluma in the cheek to smooth the under eye area. In some cases Vollure can be treaded into a tear trough area as well. 

Juvederm Ultra plus XC or Juvderm Voluma work very well in nose reshaping. Voluma is more common as the results last longer. 

Full face volumization is achieved best with Sculptra Aesthetic. Some times we will add other hyaluronic acid fillers around the mouth or under the eyes as these are places not indicated for Sculptra. 

Technically the longest lasting filler would be your own fat. After that the injectable fillers we do range from results lasting 5 years to one year. Permanent fillers are not a good idea. Your face is always changing and you want to be able to maintain a look without having to work around an implanted filler that doesn’t move with your face. 

Facial fillers have a range depending on the formulation ranging from about five years to one year. 

In the unlikely event that you don’t like your results, some fillers can actually be dissolved. Hyaluronic acid  fillers do have an enzyme called hyaluronidase that can be injected into the filler and dissolve the product. It is not recommended to use this liberally however as it can remove your body’s natural hyaluronic acid as well.
