How is a labiaplasty performed?

The goals of the labiaplasty procedure are to reduce the size and length of the labia to below the level of the labia majora and to correct any asymmetries. Small reductions can be performed in the office under oral sedation or if the patient prefers, can be performed at the ambulatory surgery center under general or IV sedation anesthesia.

Dr. Chang performs the labiaplasty procedure that he adapted from a technique first described by Dr. Gary Alter, the nationally acclaimed Beverly Hills plastic surgeon known from the television show “Dr. 90210.” 

In this procedure, a vertical wedge of the labia minora is removed. This results in a nearly invisible scar, instead of the linear scar performed by many other surgeons.  Some patients will elect to have a clitoroplasty, a reduction of the labia majora, or the diVa Laser Vaginal Rejuvenation at the same time.

  • Step 1 - Numbing cream is applied to the vaginal area and allowed to sit for 20 minutes
  • Step 2 - The base of the labia is injected with long acting numbing medication that will last several hours
  • Step 3 - The amount of labia tissue to be removed is marked out as a wedge resection.  The scar is hidden posteriorly
  • Step 4 - The marked tissue is removed
  • Step 5 - The labia is closed with absorbable sutures
  • Step 6 - Ice is applied
  • Step 7 - Additional Vaginal Rejuvenation procedures are performed after the Labiaplasty ( clitoroplasty, diva Laser )

The procedure takes about an hour.  Many procedures for the labia can be completed in our onsite procedure room with local anesthetic, so you may drive yourself to and from our office. If your treatment plan is more complex, you may need to be put under general anesthesia at a nearby surgical facility, so you will need to have a responsible friend or family member to drive you home following your procedure.

What to Expect

What to expect after a labiaplasty procedure.

The recovery from a labiaplasty similar to that for an episiotomy.  The labiaplasty procedure is not a painful procedure. Long-acting pain medication is injected into the tissue that provides pain-free recovery for the first several hours.  Some patients will take a prescribed narcotic pain medication for 2-3 days.  Patients can change to advil or tylenol after 3 days. 

Return to work after a labiaplasty procedure:  

Most patients will  :

  • Apply ice packs and frozen peas on the incisions for 2-3 days
  • Sitz baths 3x/day and after using the bathroom for 7 days
  • Whoopee cushion may be helpful in taking the pressure off the area when sitting
  • Return to work in about 5-7 days ( but will still be sore )
  • Return to light exercise walking and 10 pound weights in 5 days
  • Return to aerobic exercise and 20 pound weights in 21 days
  • Drive in 5-7 days or after having stopped narcotic medications for 48 hours
  • Avoid intercourse, tampons, hot tubs, lakes, and oceans for 30 days
  • Avoid intercourse for 30 days
  • Avoid tight pants for 30 days
  • A compression garment is required for 30 days

When will you see the final results of a labiaplasty surgery:  

  • You will feel engorged for 1-3 months
  • Decreased or sometimes increased sensitivity 3-6 months
  • Absorbable sutures are checked after 7 days and may take 14 days to absorb
  • Final shape is not apparent for 3-6 months to a year

What are the risks and complications of labiaplasty surgery?

As with any surgery, there are associated risks.  Only you can decide whether a labiaplasty can achieve your goals and whether the recovery and risk profile are acceptable to you.  Fortunately, significant complications from labiaplasty surgeries are relatively infrequent.  The most common complications are problems with delayed healing, infection from the stitches, and delayed healing.

Although infrequent, the risks and complications of labiaplasty can include

  • Bleeding, hematoma, or bruising
  • Infection particularly from the absorbable sutures
  • Widened or thickened scar
  • Asymmetry (unevenness between two sides)
  • Delayed wound healing with skin or tissue necrosis
  • Extra skin at the ends of the incisions
  • Undercorrection
  • Overcorrection
  • Unsatisfactory results that may require additional procedures.  Patients will generally be responsible for any associated facility fees.  

You will require around one week to heal following your labiaplasty to allow discomfort and swelling to fade away before returning to normal activities. Dr. Chang suggests applying a cold compress or ice pack if needed to decrease inflammation. You can not have sexual activity or use tampons for at least 4 – 6 weeks following your treatment. Patients typically have visible results in about six weeks and final results closer to six months following the procedure. The improvements of vaginal rejuvenation usually last for a long time; however, weight fluctuations, natural aging, and childbirth can impact your result.

Should I choose a plastic surgeon or OB/GYN?
When researching the aesthetic surgery market, most patients will find that a large majority of plastic surgeon offer labiaplasty. While many OB/GYNs are capable of performing labia rejuvenation, the procedure is most often considered cosmetic. An OB/GYN will be able to surgically remove any excess tissue that exists within the labia; however, a plastic surgeon like Dr. Chang will consider the aesthetics as well. Who are candidates for Labiaplasty?

Labiaplasty is often considered medically necessary when large or uneven labia minora cause recurrent infections or problems with urination or feminine hygiene. Some women are born with enlarged labia. Others have had their labia stretched during childbirth and from the hormonal changes related to pregnancy. Other symptoms which may make you a candidate for labiaplasty:

  • Painful sex and embarrassment in intimate relationships

What to expect after a labiaplasty procedure

90% of women are happy after a labiaplasty procedure because it solves a problem that is easily treatable.  Most women who have had an episiotomy can better understand the recovery.  Although the recovery is not exactly painful, it is uncomfortable to sit down for long periods for a few weeks.  The labia may remain swollen for up to 3 months.  

How long does a labiaplasty take?

A labiaplasty procedure can take between one and two hours depending on whether other procedures are added.  Common procedures for women to add include a clitoroplasty, a Diva Laser Vaginoplasty, labia majora reduction, and or a mons lift.

Does insurance pay for labiaplasty?

Insurance does not cover labiaplasty as it is considered an elective cosmetic procedure.  There is no medical reason per your insurance company to pay for the procedure.

What kind of surgeon performs a labiaplasty?

Only board certified plastic and reconstructive surgeons should perform your labiaplasty procedure.  The labia or inner lips are a sensual part of the female anatomy that deserves special attention.  Although gynecologists can also perform the procedure, their concentration has mostly been about delivering healthy babies.  The example I most often tell my own patients in consultations is who would you rather be closing your abdomen after a c-section…  plastic surgeon or a gynecologists.  

My guess is that you would rather have a plastic surgeon give you the best looking abdomen and scar possible.  The labia shouldn’t be any different.  A second reason is that most gynecologists are trained to perform a straight excision.  The more advanced techniques use wedge excisions to decrease the length and sensitivity of the scar and a more pleasing aesthetic to your female parts.    Plastic surgeons also have more advanced techniques to perform a clitoroplasty to decrease the length of an over-long clitoral hood or a labia majora reduction with fat transfers to improve the appearance of the outer lips.  

What symptoms or complaints are typical for women who elect to have a labiaplasty?

Women do not only come in for a labiaplasty for cosmetic reasons.  Very my like breast reduction procedures, the high satisfaction rate for labiaplasty procedures comes from the fact that there are many functional and psychosocial reasons to have the procedure.  These include:

  • Pain and irritation from rubbing against clothing (especially jeans and workout clothes)
  • Interference with activities including intercourse, tampon placement, urination, cycling or horseback riding
  • The appearance of a bulge when wearing certain clothes.
  • Elongated or asymmetrical labia may cause embarrassment or interfere with a sexual relationships.

What are the surgical risks of labiaplasty?

Risks of surgery are extremely rare and seen in less than 1% of cases. Like any surgery, complications may include infection, bleeding, scarring and nerve injury.  The risks involved with pregnancy and delivery are significantly higher than any risks associated with labiaplasty with an experienced surgeon.

Will you lose feeling after a labiaplasty?

No.  You will not lose feeling after a labiaplasty.  The technique that Dr. Chang uses removes only the superficial and visible layer of the labia tissue and leaves all the sensory nerves untouched.  Having said that, the labia does not contain any excitatory sexual nerves to worry about.
