Surgical Technique

Surgical breast reduction is an outpatient process and is conducted with the patient under general anesthesia.  

The principles of any breast reduction procedure is to provide you with smaller but natural appearing breasts in a safe environment.  This is why all our breast reductions are performed in an ambulatory surgery center or hospital ( not our own office ) where there are board certified anesthesiologists taking care of your well-being while I take care of your breasts. 

There are three principle techniques in performing a breast reduction.  The technique chosen depends on how much of a reduction is wanted, how much the nipple needs to be lifted, and how much extra skin needs to be addressed.  These three techniques are described below but in short, the donut technique is appropriate for only the smallest of reductions; the lollipop technique is appropriate for moderate lifts; and the anchor style technique is appropriate for larger lifts that insurance will mostly cover.  Each of these techniques have similar goals:

  • Minimize scarring
  • Elevate the nipple to the level of the under-breast crease
  • Decrease the size of the areola to about 4 cm in diameter
  • Provide a nice tear-drop shape with adequate cleavage

Breast Reduction with Donut Style Incisions -  Breast reductions using a donut-style incision is performed for very small breast reductions in patients who do not need a lot of lift.  If the areola size is too wide or the areola needs to be lifted more than about 2 cm, the donut incision will widen with time.  This means that even though the length of the incision is shorter compared to the others, gravity will widen the incision and make it more visible.

Breast Reduction with Lollipop Style Incisions -   Breast reductions using a lollipop incision is designed for middle sized reductions or for patients who need the nipple to be lifted more than 2 cm.  The donut incision around the areola is extended downward which takes the tension off the areola.  In this way, the scar around the areola is less conspicuous even though the overall length of inte scar is longer.  This technique also provides for excellent cleavage.  This technique is best for patients who need only a reduction of about a cup size.

Breast Reduction with Anchor Style Incisions -  Breast reductions using an anchor-style incision is designed for larger reductions or for patients who need the nipple to be lifted more than 4 cm.  The donut incision around the areola is extended downward which takes the tension off the areola.  An incision is then made along the infra-breast crease to remove extra skin and fat that tends to bunch up in this area.   Insurance will cover breast reductions that are generally in this category.  Insurance will generally insist on the removal of about 500 grams of breast tissue per breast for them to cover the procedure  This is about the size of a grapefruit.
 

What to Expect

Once surgery has been completed, you will be monitored closely in our comfortable recovery facility and then discharged home to begin the healing process. A small, tube-like draining device is usually inserted near the incision area to reduce bleeding and swelling. A compression bra will be put in place for a few days that will boost blood circulation to the breasts during the immediate recovery. In a month or so, the breasts will position themselves and settle and the post-surgery effects will disappear. Pain after a breast reduction:  

Breast reductions tend not to be very painful.  This is because muscle is usually not involved..  A special thing that we do with all our patients is place long acting pain medicine directly into the muscle providing good pain control for the first day.  Most patients will take a prescribed narcotic pain medication for at least 2-3 days.  Patients can change to advil or tylenol after 3 days.  Patients can purchase a pain medication called experil from the surgery center.  This provides near-complete pain relief for 3 days.

Return to work after a breast reduction surgery:  

Most patients will  :

  • Return to work in about 7-10 days ( but will still be sore )
  • Return to light walking and 10 pound weights in 5 days
  • Return to aerobic exercise and 20 pound weights in 21 days
  • Drive in 7-10 days or have stopped narcotic medications for 48 hours

When will you see the final results of a breast reduction:  

Feel engorged for 3 months
Decreased or sometimes increased sensitivity in the nipples for 3-6 months
Scars will not whiten for 12-18 months ( use scar creams for silicone sheets for up to 3 months )
Final shape is not apparent for 6 months to a year 

What are the risks and complications of breast reduction surgery?

As with any surgery, there are associated risks.  Only you can decide whether a breast reduction can achieve your goals and whether the recovery and risk profile are acceptable to you.  Fortunately, significant complications from breast reduction surgeries are relatively infrequent.  The most common complications are problems with delayed healing, infection from the stitches, minor tissue loss in areas of delayed healing, residual fullness in the lateral breast, and widened or thickened scars

Although infrequent, the risks and complications of breast augmentation can include

  • Bleeding, hematoma, or bruising
  • Infection particularly from the absorbable sutures
  • Loss of sensation of the nipple
  • Widened or thickened scar
  • Asymmetry (unevenness between two sides)
  • Delayed wound healing with skin or tissue necrosis
  • Under-correction
  • Over-correction
  • Unsatisfactory results that may require additional procedures.  Patients will generally be responsible for any associated facility fees.

Will my insurance cover breast reduction?
Breast reduction surgery is often covered by insurance.  Our office has extensive experience with insurance companies in Virginia.  We will do our best in trying to get your breast reduction covered.  If your enlarged breasts are causing you problems such as neck pain, back pain, or rashes under the breasts, your health insurance will possibly cover your surgery.  Keep in mind that most insurance companies will require a minimal reduction of about 400-500 grams per breast.  This is equivalent to the size of a grapefruit

Will my nipples be moved?
Usually, the nipples and areola will be moved up to a central position during breast reduction surgery. If a significant amount of tissue and skin needs to be removed, your nipple-areola may be cut out and moved. Dr. Chang will select an incision technique so the nipple and areola are easy to move higher. It's also possible to perform an areola reduction to reduce the size of the areolas. Your options will be explained in your consultation before Dr. Chang helps you choose the best plan to meet your goals.

Can I breastfeed after a breast reduction?
Reduction surgery can affect breastfeeding. Although some women can still breastfeed after a surgical reduction, other women cannot. It's difficult to know whether your breast reduction will interfere with breastfeeding. This is why Dr. Chang suggests that patients be done with breastfeeding before considering breast reduction surgery.

Can my breasts re-grow after surgery?
Following reduction surgery, your breasts may get smaller or bigger with dramatic weight changes. This is why it is better to be at a healthy weight prior to your surgery so that you can easily maintain your shape. Will a Breast Lift be included in my Breast Reduction Surgery?

A breast Lift if included in ALL breast reduction surgeries.  They are both similar procedures; the difference is that breast lift is mostly a skin removal surgery while a breast reduction will remove both skin and breast tissue.  The goal of both procedures is to lift the nipple position to the level of the under-breast crease.

When can I return to work after a breast reduction?

Breast reduction surgery is not particularly painful as it does not typically involve the chest muscle.  Most patients will return to work in about a week even though they may still be a little sore.

When will I see results after a breast reduction?

It is not unusual for a patient to thank me profusely immediately after the surgery because they immediately feel the relief of having the weight of the breast removed.  In addition, their breasts are immediately elevated and reshaped.  Having said this, it will take about 3-6 months for the breasts to completely settle.  And it will take about 1 to 1.5 years for the scars to lighten and flatten.

Are the results of a breast reduction permanent?

No.  Nothing about aging is permanent.  Weight changes and gravity will see to that.  Most women, however, will never need a second procedure
